This one is just for those in grades 9-12! Saturday, April 14th, we are heading to the Cities. First, we'll hit the Mall of America, and then make the short trip over to the Water Park of America. Get the forms above for all of the information.
These forms need to be completed once a year or when major medical changes occur. They are only in case something would happen so that we could get you in to the doctor. Needless to say, they are very important. All you need to do is open the document and type/check all of the blanks except the signatures, and then print and finish, or print and then write it in.
